What is motor rpm ? Is the Ammeter electronic or electromechanical. If it is an old one with a moving coil, you can see if there are current oscillations. If there are, that can be a good sign that there is a problem with rotor cage. 100 Hz oscillations can be a sign of not equal...

There is probably a problem with breaking some rod on the rotor. This can be detected reliably when motor is running under load using the Motor Current Signature Analysis method or vibrodiagnostics .

I recently saw at manual for Westinghouse High voltage motor that as starting method was given soft starter only . In your opinion, what should be structural difference compared to the motor with a DOL start .

A good question for maintenance is whether someone has been working on something at high voltage and whether there was perhaps a phase change and consequently change direction of rotation of the motor.
Is there a time delay between the release of star contactor and activation of delta contactor. Often, a small time delay is solution to solve problem if large currents occur at that moment .
Since the motor was replaced, winding connection should definitely be checked .
