I looked at AISC Design examples..... see assumption in example E.4A "Assume the column is continuously supported in the transverse direction (the y-y axis of the column)." So they are pretty much saying KLy will not govern so that is why it isn't compared in your example E.4B and they only use...
I believe in the solution they are using the KyL/ry to govern.... if you look at table 4-22 they use the value for 93.8 (y governing) in lieu of 61.1(x governing).
To your concern about KLy_eq vs KLy....see page 4-4 it states "to determine the available strength in axial compression, the table...
main key in design (to me) is understanding load transfer.... proper transfer over loads and accounting for that in the design of individual members and components so they work properly in the system is key and sometimes not properly analyzed.

100psf should probably be adequate. If you reference ASCE 7-10 they recommend 1.5x the occupancy served not required to exceed 100psf for balconies and decks in table 4-1. If you want a little more comfort you can increase the live load but I think you should be okay in terms of justifying it...
Agree with 8thstreet. I took the exams last April and there are definitely a lot of bridge questions I don't think you can get by with ignoring AASHTO by any means. Like I said before it is pretty well balanced between bridge and building in my opinion for the morning sections.
Just remember just because the question count comes out to be 20% bridge doesn't mean its 80% building. There are several questions that really don't fall under either specifics. I think it is pretty well balanced if I recall.
