It is a very referenced document when it comes to the execution of CPT tests and their stop criteria.

Pile driving formulas can be far away from reality. During school we did some comparisons between them and the WEAP analysis and they were considerably off.
All you are asking are things I learned during my bachelor degree in Civil Eng. I don't know what to say to not sound offensive. I am a geotechnical engineer now but this all is some pretty basic stuff you learn in school.

I agree with Ron. You should consider asking a geotechnical/structural engineer. Estimating a preliminary footing size is not a difficult task but you do need some information, and more importantly you need to know what you are doing.
GeoPave, I am currently using a thick casing as you described with relative success. I have now the problem that at deeper depths this casing is too heavy it cannot be lifted with the machine. I am thinking to use a crane if the time comes when I cannot take it out with the machine alone...
