Putting the lift lug on the knuckle portion of the head should be avoided.
Have you considered using QTY(4) lift lugs on the shell? Orient them at 45° and 315° directly across from each other in pairs and design appropriately. This should result in standard lift lug calculations and WRC 107...
I use the following stainless hardware when designing in accordance with ASME Section VIII, Div.1:
304 SS bolting = SA-193-B8
316 SS bolting = SA-193-B8M
I have never used any of the designations you have listed for pressure vessel design.
As far as I know there is no way to do this in PV Elite. You will need open up Code Calc and put the coupling on a cylinder equivalent to the straight flange of the head.

I see that PV Elite Please uses 1.3 times the ambient allowable stress of the material for allowable hydrotest stress. What is this based on? Is this method mentioned in ASME Section VIII, Division 1?
