Moment equations based on lateral movement of a rigid body

What is equation for moment created from a lateral movement of a rigid body. I have attached example from STERM based on a fixed-Fixed condition = M =6EI&/H^2 and I believe it is 4EI&/H^2 at pinned end and 2EI&/H^2 and fixed end. Mean Roof Height used for Components and Claddding -ASCE 7-05 -SImplified Procedure

Woodman88 and carr82775 , Thanks for the help. I actually found the mean roof height in the front glossary (ASCE 7-05), but also decided to jump to the analytical procedure (Method 2) 0.00256KdKzKtIV^2I with +- GPF-GCPI. Also deducted 10 % per footnote #4. Worked like a charm.

What is minimum cover for cast on site Tilt up Panels on site?

The UBC 97 clearly states that it is 1" for tilt up walls But in the current ACI-318-11, it seems to be vague, is it 3/4"? The walls are cast on site and not at a plant. Also where is this in the CBC-2013 or ACI 318-11?
