Is there a specific ACI code that should be used when designing concrete weirs and concrete water storage tanks?

Your best bet is to look at the ACI website. There are several committees that deal with water tanks and environmental strucutres ACI 350, 371, and 373 for starters.
 
Is there somewhere in writing that states the plastic cone holes left after a wall-tie is removed are to be patched?
 
That's a detail the code doesn't trifle with.
However, it should be mentioned in the specifications.
