Neat... but was Aghorn fined? and have they implimented the recommendations?

CSB and NTSB do not issue fines or other penalties. That is the responsibility of other agencies.

The OSHA activity can be found here

Aghorn Operating, Inc. contested the willful violations, and made formal settlement of the violations on record today. OSHA has not closed the investigation.

Thanks...

The worse job conditions I ever had was when I worked for a producer in far South TX during the 1986 oil crisis. I could find no other job. Largest independent gas producer in the USA, but clever lawyers kept them in bankruptcy for the previous 13yrs. I was there for two years trying to keep their 1800 well 1000mi pipeline system running along with 2 production engineers. Leaks everywhere and a couple of explosions every month kept me busy. When they finally hit H2S in their far south wells, I had to quit. Too much chance of walking near pinhole leaks. Fortunately oil prices were rising by 1989, so I was able to go back to Houston.
