Allowable Stress Design (ASD) is referenced commonly as a design method for bearing capacity failure; however it is not (that I've found) referenced to the failure modes listed below. As such, I'm curious if it applies to these as well.......or are these failure modes evaluated using Limit Equilibrium Methods??
1. Pullout
2. Eccentricity
3. Sliding Frictional Resistance
4. Sliding Passive Resistance
5. Uplift/Flotation

This may give you some information, but in my experience most footings are designed for an allowable settlement, since failure by other modes occur at loading in excess of what settlement criteria allow. Thus, there is little need for computing what your procedures deal with, in practice.

Serviceability (allowable settlement) may govern, but Ultimate Limit States also are evaluated using a 'design methodology'. My question is whether items 1 through 5 can be evaluated using ASD methods or are they evaluated using Limit Equilibrium Methods.
 
Why not? How foundation design was handled prior to ULS gaining popularity?
 
1 (deep foundation pullout), 3, and 4 would be ASD.

Uplift/Flotation could be different depending on what you are working on. I would suggest flotation would be more limit equilibrium.

Eccentricity? Might need a structural to answer that one.


 
LRFD methodology for substructures was 100% reverse-engineered from ASD. Anything done LRFD can be done in ASD - concepts don’t change that much.
