Anchor Flange Work Sheet Taylor Forge 1

Does someone have access to a Taylor Forge Achor Flange Wrok sheet? Or know where one could be found? I have Modern Flange Design but it didn't have that variety included. I've seen one once before and have forgotten how to include corrections for the second hub.

Any help would be greatly appreciated.
