API Addendum Effective Dates

Can anyone enlighten me on API addendum effectivity dates?
For example, if I purchase the current API 650 it currently includes addendum 3 although addendum 4 has been in print now for some time. Add 3 was written in 2003, I believe, and add 4 in 2005. Does anyone know if there are specific dates for inclusion? What is the actual current API 650 code? IS it API-650 10th edition with addendum 3, or 4?

The 11th Edition is out now. See the Foreword of Addendum 4 for effective dates of it- "may be used beginning with date of issuance shown on the cover page...becomes effective six months after the date of issuance" and either may be used in between.
