Formula 12.14-6 defines D as "effect of dead load". Does anybody know if that is the same D as in 12.14.3.1.3 combination #8 ? Isn't D the Dead Load ? what is the "effect of dead load"?

All of your load cases in all of ASCE 7 combinations are "effects", not loads.

In other words, you apply the loads on the structure in the various directions that they occur (lateral, vertical) and the effects are the axial, shear and moments that result.

Thus, the combinations are really combinations of effects, not loads. But to analyze it you apply loads to the structure.
