ASME Data Report Form Templates 2

I need to fill out an ASME Data Report that I have never had to use before, and was wondering if anyone knows where I can find an excel or word version of ASME Data Forms (particularly a P-3). So far I've only been able to find pdf versions of the forms. I could always create one myself, but it would be nice to save some time.

FYI.....(For Y'alls Information) I just updated the site and ALL ASME forms are now updated to the 11 Addenda... Links provided above will not work because forms have been placed in newly named folders.......
