ASME PTC 4.1 Short Form

I just received my copy of ASME PTC 4 2008 and was surprised to find it didn't include a short form for the so called abbreviated efficiency test like PTC 4.1 did.
Is the short form no longer used?
Where can I get a copy of the short form from PTC 4.1?
I can't find a good copy anywhere on the NET. There are a few books online that show the short form but they are of poor quality.

Any suggestions?
