ASME Tubesheet WPS/PQR template

Just as the title implies, I am looking for a Word or Excel template for tube sheet welding. In the world of everything electronic finding a template would save me some time.

The only thing I could find was a tube sheet rolling/expansion PQR,

To clarify in advance I am in no way looking for a program that automatically creates Code WPS/PQR's

If the tubesheet welding is going to be used on ASME Section VIII, Div 1 or Section I components, the welding must be done to ASME Section IX. Use Section IX as a guide for qualification of the tube to tubesheet weld joint and follow either Section I or Section VIII, Div 1 for weld joint design details.
