NFPA 13, 2007 clearly states:

8.15.8.1.2 Sprinklers shall be required in bathrooms of limited care facilities and nursing homes, as defined in NFPA 101, Life Safety Code.

From the 2009 edition of NFPA 101,Life Safety Code:

3.3.82.2 Limited Care Facility. A building or portion of a building used on a 24-hour basis for the housing of four or more persons who are incapable of self-preservation because of age; physical limitations due to accident or illness; or limitations such as mental retardation/developmental disability, mental illness, or chemical dependency.

3.3.131.2 Nursing Home. A building or portion of a building used on a 24-hour basis for the housing and nursing care of four or more persons who, because of mental or physical incapacity, might be unable to provide for their own needs and safety without the assistance of another person.

The only thing I can think of is:

IBC 903.2.5 Group I. An automatic sprinkler system shall be provided throughout buildings with a Group I fire area.
Exception: An automatic sprinkler system installed in accordance with Section 903.3.1.2 or 903.3.1.3 shall be allowed in Group I-1 facilities.

903.3.1.2 NFPA 13R sprinkler systems. Where allowed in buildings of Groups I-1 and R, up to and including four stories in height, automatic sprinkler systems shall be installed throughout in accordance with NFPA 13R.

And 13R-07 6.9.3 allows them to be excluded from bathrooms that do not exceed 55 sf.

Keep in mind, they may be other provisions of the code, like height and area, that may required an NFPA 13 system. Or even the owner or its insurer may be requiring it.
