Belt loosening / slippage - dynamic issue?

My collegue is involved on a problem identification of a "belt loosening / slippage problem leading to failure" in a motor (11kW power) driven screw compressor after a few (20 - 30) hours of operation.
Is there anything else he needs to consider apart from the static design, contact angle, etccc? Say, whether a torsional analysis is required to see whether he is operating in some out of phase modes? Could this also be point of concern? Any other check points.

I'm not sure I understand the question, but..
what type failure?
vee belt or synch belt?
what size?
what tension ?

The "belt loosening" and "belt breakage" are the failure modes observed in this case. The belt is of Vee type and belt tension is set with a tensioner.

I suppose the belt drive could be grossly undersized for the application, or the screw compressor could be jammed, but if its an OEM design I'd get to understand the installation procedure. SHould be in the manual.
