Dear all. Hello.

Have you ever heard about boxed-up format, which is completed right before closing the manhole of an equipment's?

During overhaul, some of the equipments has been opened and cleaned and then after checking all of the internals, a check-up format is completed and then manhole closed.

Is there any templates of Total or whatsoever?

In pre-commissioning and commissioning activities for start up of a new plant or principal equipments for the process after a general maintenance, we used to handle a format where the Process Engineering, Inspection team and Operations supervisor verified and signed that the all internals as set as the design drawing set, have been 100 $ clean and no tools, rags bolts  and others were left inside. After that the equipment should be closed.

These documents belongs to the commissioning procedures.

Right, what about during normal operation???

Use the same ones or ask your client.

If he says that's your job, then use what you have form commissioning, but cut out bits you don't need.

If this is Total job then they should have a procedure or template in their voluminous library of specifications and forms.

You can use the same format, for the internal inspection before the manhole be closed.

key is that everything be in the right place and nothing were left.  many chemicals plant has foung unpleased surprises into equipment on troubleshooting for bad plant star up.
