Causes of fire in a heater (oil, emulsion) treater. especially vertical

Hi,
what are the most common causes of fire on a heater treater, could it be that:
1. the water levels falls to much that the oil level comes in contact with the fire tube which is supposed to be inside the water level?
2. Or the fire tube was in the oil level in normal operations and somehow, flame came in contact with the oil by burning through the fire tube
3. or the temperature sensor mal-functioned and did not shut the igniter or pilor when temperature got too high

those are just thoughts going through my mind, does anyone have experience with heater treater fire incidents or just knowledge of the causes of fire?
