certificate for ASTM

I'm a forging plant in China. Now one of my customer asked for a certificate for ASTM. The forging is C1045 rolled ring. Is there any format for this certificate?

There isn't a specific format, but the individual ASTM standards do identify the items that must be listed on a certificate.
