CLOMR Application and MT2 form 1

I am working on a CLOMR application for a client. My report is based of the requirements in the NFIP 65.12 regulations. I am attaching my MT2 form as attachment to my technical data package. I am unsure whether it is the applicant (client or contractor-me in this case) who fills out the MT2 form or if it is the Community that provides us with a filled out copy since the community engineer is supposed to provide us with a certification that no insurable structures are impacted.

Also What is the fundamental difference b/w MT2(CLOMR/LOMR-F) and MT1(LOMA)? I also always wondered what MT stood for.

I typically fill ou the MT2 forms, and then send them to the community for review. They'll review, and if they agree with the submittal, they will sign (where required) the forms. I believe the engineer has to fill out the forms because it must be sealed.

As far as MT-1, I believe those are used simply to remove individual (or small groups) of structures from the floodplain (as opposed to revising the map). When these are submitted, I think FEMA usually issues a letter stating that the structure is not in the floodplain based on it's Lowest Floor Elevation.

Would you know wherre I can find a completed CLOMR application online, which I could use as a go-by? I looked up google did not find any pdfs that had the completed application package.
 
You can go to:


You can either enter the FIRM panel number (if you know it) or do a 'MAP SEARCH' (close to top of the page). Once the results are returned, click the + sign under 'SHOW LOMC'. This should show a list of PDFs of any CLOMR/LOMR/etc. that have been completed on that map. Usually they will contain the letter of determination, MT forms, etc. etc.

If there are none that have been completed on the map panel you're working on and you just need a go-by, just search any random map panel until you find one. (these won't have any of the technical narrative, usually just the forms).
