Concrete Mix Design

Dear All,

I have an enquiry regarding concrete mix design. Usually how do you guys review the submitted concrete mix design by the Contractor? What items do you check? Other than concrete mix design, what else do you usually require them to submit?

I require:
source traceability on the aggregates and gradation compliance with ASTM C33
test results on the aggregates for reactivity, durability and soundness
traceability of cement source
mill certificate on the cement
test results on the cement for set time and compliance with ASTM C150
record of service and compressive strength statistical data for the specific mix design

For the specific mix design, I look for the water-cement ratio and aggregate sources to compare to the submittal of the backup info.
