CSA C22.1-21 Diagram/Table D17J - Bonding Conductor Installation Question

In diagram D17J, single-conductors are installed in a concrete duct bank. The phase conductors are in separate conduits. There is also a parallel set of phase conductors.

I found some rules that seem relevant to this question:

In these rules, it specifies that the bonding conductor should be installed with each group of parallel runs. However, it's not clear about how to install the bonding conductor if the group of phase conductors are in separate conduits.

This diagram is for ampacity calculation only and it is the same as per NEC 2020 art.311
For bonding -or grounding [earthing] rules see Section 10 - Grounding and bonding-or art.250[NEC].
In my experience, a grounding cable will follow the duct bank and a connection in each manhole to a grounding bar, for all metallic parts mounted here-cable support, usually. A 4/0 copper conductor will do this job, in direct contact with the environmental earth of the duct bank.
