Cutting Large Circular Holes into Tubing through Both Walls

I am quoting a rather large order in which I have to some 4" x 4" x 1/4" carbon steel tubing that I need to get a 3.06" +/- .010 hole through. If that isn't bad enough, take the same description and move the center line of the hole to the outside edge of the tubing and machine though. Any thoughts on tooling out there to do this operation with? I have 4000 pcs. of each part to make.
 
That's not a bad idea. Do you know of any companies that I could trust to do the work and do it right? I have been bitten before. I am located near Chattanooga, TN.
 
The first hole you could probably trepan. If I understand your second hole it sounds like you would want to mill it from the side.

Alternately, the holes could be rough plasma cut and then finish milled or bored.

Hougen Rotabroach or similar cutter. It will probably be a special in that diameter and length.
