Dealing with impact rust

I'm working on a bridge rehabilitation project that includes painting several riveted built-up steel girders. In several locations the girders exhibit areas of moderate to heavy pack/impacted rust. I've been researching various methods for how to best address these areas and thought I'd check here to see what others have done. Given the complexity of the issue, and the difficulties associated with creating a long-term repair, I expect there will be quite a few different perspectives.

So my question is, how have you seen pack rust dealt with in the past? Specifically, what repair procedures and materials have you seen specified?
