Diagonal vs. Conical Failure of Test Cylinders

I am interested in your opinions and experiences regarding the failure mode of test cylinders. Should concrete test cylinders with compressive strengths in the range of 3000psi to 4500psi break as a diagonal failure or as a conical failure? At what strength ranges are diagonal failure types most likely (assuming that the cylinders are capped and loaded correctly)? We almost always see conical failures for concrete breaking above 4500 psi.
