Anybody have a generic ECO form that they are willing to share?

I have been assigned the task of creating one, and of course would rather not do it at all (if I can help it).

But, just in case no one has something generic that they are willing to share, I'll also accept suggestions for what you folks think ought to be on an ECO form.

What industry or end product are we talking about? An ECO should cover the following questions:

Who wants the change?
Why is the change required?
What is changing?
When is it changing?
What happens to the old parts?
Who needs to be made aware of the change?
Who approved the change?
Who completed the change?

Sequential serial number of the change for tracking purposes.

To add/modify to the above, capture the current configuration (for legacy sake and in case you do end up going backwards) and then indicate the new changes. I have seen this shown under "current" and "new" titles or "Was", "Is". In addition to change cost, if you are working in a manufacturing environment, you can have a location for capturing affect on product cost (increase, decrease, no change). Along with parts disposition, you can look to capture effectivity levels for the ECO. For example whether or not you need to rework items in process or even pull from finished inventories to rework product. We also note whether or not a change should be mandated into product service/repair and whether or not it is covered under warranty.

An ECO is a hugely important document that affects many areas of a business. Try to get feedback from those who will be affected by ECO's in order to capture the information that they will need/use.
