Electrical Form Board Approval Checklist

I am looking for a checklist, or form, whereas an avionics form board is approved for use on a production floor.

Example would be:
Drawing #, Drawing Revision Level, Drawing dash number (i.e. EH-400-3456-501 rev C)
Drafting Drawing Approval
Engineering Approval
Dimensional Check
Operations Approval
Quality Approval

Do you have a form board approval checklist that you could share?

Confusing post...

Does this have to do with the power supplies to machinery on an aircraft production floor?

Or does it have instead to do with certification of avionics wiring? If the latter, better to post in this forum
