Examples of PSV Failures due to Lack of Maintenance

Hello,

I am preparing a presentation on PSV maintenance and would like to include an example or two on incidents due to lack of maintenance/recertification on PSV. I tried CSB website but the ones I found were more on no PSV or PSV isolated from equipment. If anyone can share an example of such incident / failure, that will be greatly appreciated.

I know a case where the PSVs were not maintained and small steam leakage caused scale on the seats. This was a 90MW power station and when it tripped 1/2 the PSVs did not open. There were pipe component failure (burst) in the turbine hall.
