2. Which code specify which all shaft or duct needs to be fire rated/ fire wall or it is upto the designers choice?

NFPA 101 which is the life safety code for occupancies such as industrial, commercial, institutional will state the fire rating requirements.
Your statement about bulk head openings, to what situation are you referring as there are different types of bulkhead?

When you research code, you will learn fire walls cannot have fire dampers. fire rated walls can have them.
whoever did the life safety plan should tell you what exactly each wall is.

I know is pedantic, but the term firewall gets misused all the time.

Thank you Chicopee & Energy,
Yes definitely , i was talking about fire rated wall :)...
Well some details about type is mentioned in NFPA 90A...well its the mixture of NFPA and local AHJ...i will give a dig

Since I know a little bit more about your inquiry based on the additional comments, NFPA 101 will not the correct code as it is a life safety code.

Learn the difference between a fire wall, a fire barrier, and a fire partition. Fire walls as defined by the IBC shall NEVER be penetrated, even with fire dampers.
