I am currently involved in developing a learning program which will be used to help fluid power engineers better understand the design & failure modes of safety related parts of fluid power control systems. I am looking to see if anyone has any examples of fluid power control systems that have failed to danger, in particular from failure of control valves (no brand names need to be mentioned) in circuits with accumulators. Any information such as what happened, the cause found, near miss or accident, possible web references, personal experiences etc will be of benefit. Much appreciate if you can assist with any examples we can use in case studies & even better if I can verify for the learning program. May also publish them on a website at a later date.
