Well, here are some of my favorites:

"Forensic Structural Engineering Handbook", by Robert Ratay,
McGraw-Hill, 2000, ISBN 0-07052667-2

"Structural & Foundation Failures", by B.B. LePatneu and S.M. Johnson, McGraw-Hill, 1982, ISBN 0-07-032584-7

"Concrete Failures", by Jacob Feld, John Wiley & Sons, 1968
SBN 471-25700 1

"Lessons from Failures of Concrete Structures", by Jacob Feld, ACI Monograph No. 1, 1964, Preprinted by University Microfilms, 1979

"Forensic Engineering - Proceedings of the 1st Congress", edited by K.L. Rens, ASCE, 1997, ISBN 0-7844-0277-9

"Structural Failures: Modes, Causes, Responsibilities", ASCE, 1973

You might try looking at bookfinder.com for copies of the books or calling ASCE.
