According to ASME Sec.I PW-17 It is realized that forged flat heads shall be manufactured as forged disc and utilizing forged bar is not allowed. In addition, in code case 2697-1, there are some requirements such as reduction ratio of forged disc. I want to know if using forged discs for flat heed is mandatory or not.

I'm not sure about ASME Section I, however in ASME VIII Div 1 there are many flat head geometries where the use of flat rolled plate is permitted. And then there are specific sketches and geometries where rolled flat plate is not allowed.
Perhaps the notes on Figure UW-13.3 in Section VIII will help you.
 
A brief history:
At 2011 - Code Case 2697 was approved but was not adopted into Section I.
In the 2015 Edition of ASME Section I there was no reference for forged head. So the manufacturer was allowed to use Code Case 2697.
At 2016 - Code Case 2697 was Revised.
In the 2017 Edition of ASME Section I, New paragraph PW-17 was added, which allowed the use of forged head. But, there were no limitation regarding the use of Plate, bar or rod.
In the 2019 Edition of ASME Section I, paragraph PW-17 has been revised. it is now prohibited to use Plate, bar or rod in lieu of forging (Record Number 16-1932)

Thanks alot for your answer. PW-17 is just about flat heads with hub flange, so other flat heads are allowed to be produced from forged bar, am I right?
 
The way I see it is as follow:
If you want to design or use a flat head with integral hubbed flange, the hub shall be intergally forged or machined from a forging and you are not allowed to use any other raw material for this kind of construction.
For example - you can use SA-965 (Steel Forgings, Austenitic) but you can't use SA-479 (Stainless Steel Bar)

If it is just a flat head, without integral hubbed flange just make sure it's meet the requirements of PG-5 - PG-9
