We produce an electronic automotive product and I'm currently completing a DFMEA. I'm trying to ensure I capture all potential root-causes to the failures modes. As part of this I'm concurrently doing a 5-why root-cause analysis for each failure mode. But can anyone suggest any other structured approaches to ensure I capture all causes? I was thinking about looking at the lifecycle of the component and identifying any potential hazards at each stage - is there a formal process for this or just brainstorming?

Perhaps you can analyze the risk from perspective of the top event(s) - in that case Fault Tree Analysis (FTA) would be a good choice. Refer to IEC 61025 for more information.
If the product has any control function, maybe it can be a subject of CHAZOP (Control HAZOP).
