Help with adding a custom menu to SW 1

Is there anybody who can send me a piece of code which will add a menu to SolidWorks, menu that I can further modify or customize?

The SolidWorks API support website ought to be the ideal place to find what you're looking (many VB or C++ code samples). I recall seeing a sample that might exactly what you're looking for (or something that's at least close) on their website.
