History of Control Reliable (ANSI B11.19)

I have ANSI B11.19 1991, and I believe this was the first year that Control Reliability was mandated, and defined. Please correct me if I am wrong.

I was under the impression that it was ANSI/RIA R15.06-1999 that brought this term to the forefront, but perhaps it existed before.

If a 1991 standard included that term, I wonder how (if?) anyone actually managed to accomplish it back then! We have enough difficulty with it today.
