Hot Work and Fire System Impairment Tags

Most of the property insurance companies have some good hot work permits - FM's is the best. Anyone know where i can get a good hot work permit to give to clients? I cant find anything in NFPA 51B.

Same question for tagging out a fire system/fire control valve. I recall seeing a very simple tag example in the CFC (IFC) awhile back but it didnt have much detail.

Keep in mind, i am aware of the needed precautions, I just would like a good hanging tag for both. Thanks.

The links you posted are proprietaray (FM, risk logic, etc). I supposed i could use these, but i was hoping for a generic form not tied to an insurance company, or other organization to avoid any TM issues.
