Dear Engineers,

In case that a pressure vessel or another static equipment periodic inspection was lapsed and was not done on time for many reasons such as Force Majeure, lack of maintenance contractor, not enough budget; which is the role of the plant inspector here? and what he should do?

And how to ensure the equipment integrity?

Sounds like a more general culture problem in the organization.

Look for a new job at a reputable company.

List the equipment in the process plant without inspection in the last 4 years, and write a report to the company that they have to do it at  once.  .It means you have to make a general maintenance and inspection , force by safety concern

Get a good lawyer. Same thing happened near me - I know about it because it killed the boiler manager and crushed two people to death in a building around 400 feet away. The investigation also brought in the company who did the temporary just to get going "Replace this ASAP" repair job that was left as-is for several years. It's rare, but it does happen.

If that's the attitude at the company, to let things slide, it's possible you may be let go for doing the right thing. Hence contacting a lawyer first.

In this situation, you can do couple of things to get peace of mind.It’s all about safety of personal and equipment.
1. Find out the criticality of the pressure vessel.
2. If there is operating histogram, and inspection records, do a FFS assessment. A FFS can determine if the vessel is acceptable for continue to run.

FFS is a bit pricey but don’t depend on force majeure and other factors you stated.

Could this be an operatios manager padding his/her profit margins by cutting back on inspectio and maintenance? Have seen this often enough with bad results - usually left to the poor slob who inherits the outcome after the former manager is promoted for making the profits look so good.
 
Try risk based inspection and follow the risk matrix accordingly.

The complimentary organization to the pressure vessel and boiler inspection guidance of SAFED is the National Board of Pressure Vessel Inspectors. They publish a number of relevant technical articles.. Pay particular (but not exclusive) attention to deareators, their service contains all of the most significant factors that warrant regular inspections.
