Joining skewed faces via a "Jumper"

I have a pipe frame that I will be simulating using Autodesk Simulation CFD and it has multiple inlets and outlets.
These inlets and outlets go to panels (not shown) below. I would like to directly connect/jump these inlets and outlets to bypass these panels.
The created solid will be used as a resistance value to represent the panel.

The problem is that the inlets and outlets are not in the same plane which complicates the geometry. I would like these jumpers to be tangent to the elbows to avoid meshing problems in CFD.
I tried doing this with a loft, but this does not maintain the cross-sectional profile and the part becomes thinner in some areas.

What is the best way to go about doing this, or is there a better way to handle the simulation complete?
