Liner Repair of fertilizer storage tank

Contractor attempted repair on storage tank containing fertilizer with a polyvinylchloride liner. The patch delaminated from the vessel at some point before or during initial fill leading to another tank clean out

My own understanding of linings is such that they likely did a poor job prepping the liner in place for the patch repair to properly adhere. This seems likely because other repairs inside the tank did not fail and we made a special point to confirm initial fill practices. Are there any standards or best practices one would recommend for this second go around?

I'm a little frustrated personally because we had time and money to replace the entire liner but the contractor didn't seem to want to do that and felt the liner integrity was secure.

What was the adhesive used. Prep is important and but you need to use the correct adhesive as well.
 
The supplier/vendor of the patch or liner usually has required practices.

Surface prep of both the steel and the PVC overlap is critical as is temperature and humidity.

Internal picture would be good.

How was the patch undertaken to deal with the thickness change?
Was the repair procedure reviewed and approved by you or an experienced liner engineer?

You also have to confirm that the original coating material can accept 'patching'; some coating materials cannot. Was the contractor instructed to use a particular coating material? or was he to determine the patching material to be used? Coatings are a real specialised item and depending on the circumstances, may require the input of a specialised coating consultant. [pipe]

Original patch failed due to incorrect bonding on substrate due to ineffective cleaning when flooding the liner is the current theory.

Found out this liner repair was not the cause of the “original” leak - the cleaners who flooded the liner originally missed an area in the center and the thought is that product left between the liner was pushed out while filling. Though this delamination would have hurt us anyways.
