I'm fishing for some advice or a format in creating a "living document". I work for a small firm and typically do 1 and done projects. After the report has been submitted, our work is typically done with the exception of an occasional questions or miscellaneous issue. However, I'm now managing a large project in which designs are superseded as new information becomes available. I need some way to document and maintain a record of decisions that are being made as the project progresses. Some decisions superseded others, and the history of the decision making process is of course important.

Any references or suggestions on how to do this without creating confusion are appreciated.

if it is CAD drawings you can do revisions with revision comments on the drawing along with dates.
Otherwise, maybe excel or access with columns for comments, revision dates, etc.
