Looking for reference on max holes size for 3/4" A325

Somewhere I saw it was 13/16" is the max hole size for 3/4 but I can't find the reference. Anyways that max hole size for any structural bolt is 1/16" oversized correct?

Are you steel to steel, or steel to concrete? Steel to steel I think its 1/8" bigger for oversized than standard. For steel onto concrete (anchor bolts) you can go bigger but provide washer plates as needed.
 
The max is whatever the EOR says it is. There are some guidelines, standard practices, etc., but it's ultimately up to the EOR. Why?
 
Std. bolt holes are indicated in AISC 360-10 Table J3.3 For a 3/4" Dia. A325 the std hole dia. is 13/16 (i.e. bolt diameter + 1/16") In calculations involving bolt holes an additional 1/16" is customarily added to the bolt hole dimension to account for local damage due to drilling of the hole. (I thought this was in AISC as well but I can't recall where).
