Manufacturer's Design Report (ASME VIII division 2) 1

Dear all,
I am facing some difficulties in preparing a Manufacturer's Design Report, as required by ASME Section VIII, division 2. I would really appreciate if anyone could provide me a sample of the Manufacturer's Design Report, since my remaining time for ASME Stamp certification is extremely limited...

I suspect the MDR is a confidential document, hence people is reluctant to post it for everyone's view. However, the code is so specific in filling the A forms, that I am having trouble understanding your problem. Can you please be more specific of where is your snag, perhaps we might try to fix it.
