Minimum height for equipemnt on distribution poles

I'm trying to determine the minimum height requirement for mounting fused cut-outs on a 13.2kV distribution pole that is part of a customer owned overhead distribution line on a facility property. The particular pole in question is located on the grass alongside of a roadway within the facility premises.

Would this minimum height requirement be one outlined in the tables in the NESC or are there other requirements that would be applicable to this application?

The particular pole in question will have fuse cut-outs with a group operated switch located just above the cutouts.

Is there typically a minimum spacing requirement or best practice for the spacing between equipment on a pole (I.E. spacing between cut-outs and group operated switch, etc...)

See NESC 011A8. Sounds like it may be a similar system? But customer owned means a different authority is involved, so you might want to ask the inspector.

I'm still learning the layout of NESC. Is 011A8 a particular section? I am looking at an older addition and cant seem to correlate that number to anything.
 
011 is the Scope in Section 1. See 011 A in older codes.
