Need info to model a 6" SWEEPOLET!

Hi everyone,
Anybody have dimensions for construction or location of a 3D model for a SWEEPOLET? I have a 26" OD pipe that is connecting to a 6" pipe via a sweepolet and I am trying to model that in solidworks. Ideally I was hoping to find a toolbox part or something to drop in.

Any help or info you can supply would be greatly appriciated!
