Need to know how to create a drawing format with a permanent table for every to use in each drawing.

I want to have one drawing format that will have a specific name that will always be used as a quality plan as the last sheet in our drawings. I need to know how I can add a table to this format save it off into the database so anyone can add this sheet to a drawing and be able to edit the cells.

I have loaded a format with a specifice name and added the table and then did "Save Sheet Format" and when I created a new drawing and added a sheet using this special format it loaded but the table was missing.

In addition to "Save Sheet Format", do "File/save as/*.drwdot".

What type of table?
Tables are not added to the Sheet Format, only the sheet.
