it's possible to produce a note as the image attached or I need to create my custom symbol ?

You can use a single cell tabular note and underline the text (surround the text with <U> tags).

more than a thousand ways to to this
create a text file with that text on your desktop - drag the text file from the desktop onto the Drafting.
since nx6 create the text copy it in to yor reuse lib. - drag it were you want.
since nx1 create an custom symbol ....
save the text in a part file ond import the file when you need
...

If you use the tabular note suggestion, enter your note as one line of text and change your text wrap options in the cell. Resize the width of the cell to what you desire and the lines should wrap (depending on your cell height options).

Anyway, to reproduce like my image attached, the solution is to create my custom symbol.
Rectangle sketch and text inside underlined.
