We designed an o-ring face groove for a .103 (inch) cross section diameter o-ring. The design groove width was in the range of .139 inch. But now I see that tooling is almost non-existent to machine this size groove (remember face seal.) Does no-one use this size o-ring for face seals? I see a plethora of tooling for the obsolete DIN size -- 2.5mm cross section dia.-- .125 inch wide groove. What am I missing?

You don't have to trepan the groove direct to size on all three faces; few cutters are optimized for doing it that way.

You can use a narrower cutter and some radial feed or cross feed and/or another plunge, depending on the machine.
