Seems to me the injured and killed were on top of the deck and went through. A trajedy in so many ways.
It can be very important to have eyes under the deck if there were workers down there. Had someone been in place to see the shifting or initial start of the shoring failure, the loss of life may have been avoided.
I hope a full investigation reveals the root cause such that future trajedies can be avoided and every worker can return safely home every day.
The failures in the past few months must serve as learning tools to all of us in the industry, whether engineer or contractor, and we all need to keep our eyes open for mistakes or flaws in logic that may mean problems.
My sympathy to the families and communities affected.
