PDM Standard search card

Working to develop a search card, I am trying to make check boxes work to enable me to filter by extensions if checked. Is this possible? For example, 3 check boxes 1) sldprt, 2) sldasm, 3) slddrw. I would like to check 1 or 2 or all three which would be the primary search criteria, the I would fill in one or more other fields in the search card and it would search by those fields, but only the filtered extension I checked.
I know I can type in an extension using the "name" variable field, but It gets clumsy to type in the full extension every time I search
