As per 31.3, in wall thickness calculation formula, for the coeficient Y, a table is given for t < D/6.
And for " For t ≥ D/6 or for P/SE > 0.385, calculation of pressure design thickness for straight pipe requires special consideration of factors such as theory of failure, effects of fatigue, and thermal stress"

I want to know how and where these factors are to be incorporated to calculate the thickness.

Like for 2" Sch XXX the thickness (11.07mm) is > D/6

Read the code and you will know. Means, equations and graphs for calculating these effects in terms of required wall thickness required are also given in B31.3

I hate Windowz 8!!!!
